Position: Care Co-ordinator (Occupational Therapist)
£23,000 with the opportunity to progress to £28,000 depending on qualifications and performance
ENA Care Group have an exciting new Care Coordinator position available. This would be ideal for somebody who has knowledge in Occupational Therapy.
ENA Care Group are looking for an enthusiastic, reliable and dedicated individual to join our team in Welwyn Garden City.
The candidate should have Occupational Therapy experience whether working in the setting or within an education environment, as this role will involve client visits for assessments.
Driver’s licence is essential as well as own car if not entering into salary sacrifice car scheme. An enhanced DBS check is required.
We are looking for a suitable candidate who can work full time hours along with on call hours on a rotation basis.
Duties and responsibilities
To work independently, managing own caseload and diary
To assess client’s needs and agree care packages.
To support the Care Manager of the care team and the clients in the charge of ENA Care Group, ensuring a positive experience for all in the endeavour of placing carers with clients.
To negotiate fees and rates for new clients in line with Company policy and procedure, always seeking approval from the Care Manager or Director prior to agreement with the client.
To ensure that all clients have up to date personal care plans that are accessible to the whole team and provide the standard of care expected of the company. The care plans are to be reviewed regularly.
To report changes observed or information communicated regarding a client to the Care Manager , whether physical, emotional, social or spiritual.
To supervise and manage a team of carers within a geographically defined area.
To contribute to the protection of vulnerable adults from abuse.
Undertake independent therapy assessments with a focus on Reablement in order to plan, implement and evaluate treatment and care for people in their own homes
To carry out home visits to clients in line with Company policy and procedure.
To undertake care work in an emergency situation. To be flexible, enthusiastic and have a “can-do” approach about all aspects of the job.
To participate in providing care when in non-emergency situations.
To cover the out of hours telephone on rotation with the team.
To act as cover in other teams if needed and as directed.
To perform any other duties as reasonably required
